Under-Eye Consultation Worksheet

Reception at Dr Refresh Med Spa in West Hollywood

Prepare for an under-eye consultation by writing down what you notice, what you want to change and what you need explained before choosing a treatment. Bring your treatment history and leave space to record the clinician’s assessment. You do not need to arrive with a diagnosis or decide on an injection, device or procedure in advance.

Our Dr Refresh under-eye consultation worksheet gives you a place to organize that conversation. It follows five steps: describe the concern, list earlier treatments, ask about the assessment, discuss the proposed options and write down the next step. Save this page or copy the tables into a private note to bring to your appointment.

For an overview of the treatments discussed on our website, read the under-eye rejuvenation guide. This worksheet serves a different purpose: helping you ask and record questions about your own consultation. It does not determine which treatment is appropriate for you.

Describe what you see before naming a treatment

Start with the change that brought you here. Write it in everyday language: “I notice a shadow in photographs,” “the skin looks different when I smile,” or “one side looks different from the other.” These are examples of observations you might bring, not explanations of their cause. Leave the explanation open for the clinician to assess.

If several concerns matter to you, put them in order. A useful opening sentence is, “The first thing I would like to discuss is…” Add what would make a consultation worthwhile, such as understanding the concern, comparing options or learning why a particular approach may not suit you. You can ask for information without committing to treatment.

ObservationYour notes
What do I notice in my own words?Write the concern without assigning a cause.
When did I first notice it?Record an approximate date if you remember.
What is my main goal?Choose the concern you most want explained.
What am I unsure about?List questions to revisit during the assessment.

You can also note whether your concern is about appearance, comfort or both. Tell the clinician about symptoms rather than assuming they belong to a cosmetic treatment discussion. If you are uncertain which type of appointment to book, contact Dr Refresh and describe the purpose of your visit. Keep personal medical details for the practice’s appropriate consultation process.

Bring a clear treatment history

Write down earlier treatments around the eyes and elsewhere on the face, including the approximate dates, treating practice and product or device names if known. If you do not know a name, write “unknown.” A saved receipt or record from the earlier provider may be more useful than trying to reconstruct a technical name from memory.

Keep a separate list of medications, supplements, allergies and medical history to discuss privately with the clinician. Ask the practice what information it wants before the appointment and how to provide it. This worksheet is an organizing tool; it is not a reason to change a medicine, supplement or treatment routine on your own.

For each earlier procedure, note what you remember about the experience and any follow-up you received. You do not need to judge whether it was successful before sharing the history. If you have concerns about a previous treatment, identify the questions you want answered and bring the information you have available.

Questions to ask during an under-eye consultation

Ask the clinician to explain the assessment before moving to a treatment recommendation. You might say, “What do you think accounts for the concern I described, and what makes you think that?” Then ask which parts of the concern the proposed option is intended to address. Record the explanation in words you understand.

Keep the assessment and the treatment choice separate in your notes. One line can describe what the clinician observed; another can describe the option discussed. This helps you see whether the recommendation answers your main question. If the connection is unclear, ask the clinician to walk through it again rather than filling in the missing explanation yourself.

If you have read our under-eye collagen induction service page or dermal filler information, bring your questions about those pages. Reading about an option does not establish that it is the right choice for your concern. Ask the clinician to explain its relevance to your assessment.

Discuss risks, alternatives and the practical plan

Ask for an explanation of the proposed treatment’s risks, limitations and alternatives before deciding. Write down the exact treatment or product name discussed so you can review the appropriate information afterward. Ask which information applies to the particular area being considered, rather than relying on a general description of the product or procedure.

For injectable fillers, the FDA’s patient information recommends discussing the injection site, risks, provider experience and the specific product’s patient labeling. Use those topics to guide your questions if a filler is proposed. This reference is not an endorsement of any under-eye product, treatment or use.

Ask who would perform the procedure, what their role is and who handles follow-up questions. Request the practice’s instructions for contacting the team about concerns and for seeking urgent help when needed. Record those instructions directly from the treating clinician. Do not use a general blog article as a substitute for your individual care instructions.

Bring practical constraints into the conversation: an upcoming event, work commitments, travel or questions about returning for follow-up. Ask the clinician how those circumstances affect the proposed plan. Request an itemized estimate that distinguishes the consultation, proposed treatment and any discussed follow-up. This page does not quote a current treatment price or promise a recovery timetable.

Decision questionAnswer to record
What is being proposed, and why?Write the clinician’s explanation and the exact option.
What alternatives did we discuss?Include the option of not proceeding.
What risks and limitations apply?Record the explanation and any patient information provided.
What are the practical arrangements?Note the estimate, timing and follow-up contact.
What do I still need explained?Keep unanswered questions visible before deciding.

Leave with a next step you can explain

Before leaving an under-eye consultation, try summarizing the discussion back to the clinician. For example: “My understanding is that we discussed this concern, considered these options and agreed that the next step is…” Ask them to correct anything you have misunderstood. The next step may be another conversation or assessment rather than a procedure.

Write down what was decided, what remains undecided and whether there is anything you need to provide. If you want time to think, ask how to return with questions. If an estimate or plan changes later, ask for the updated explanation so your notes reflect the current discussion. Keep the clinician’s written instructions alongside your own notes.

You can use our questions to ask before booking a med spa visit for broader provider and practice questions. Keep your under-eye notes focused on the concern you want assessed, the explanation you received and the choices available to you.

Plan your visit to Dr Refresh in West Hollywood

Dr Refresh is at 1106 N La Cienega Blvd, Suite 206, West Hollywood, CA 90069. If you are traveling from elsewhere in Los Angeles, confirm the appointment details and any arrival instructions with the practice. The location is West Hollywood; this article does not describe a separate Los Angeles office.

When you request an under-eye consultation, explain that you would like to discuss your concerns and understand the available next steps. Ask what records to bring and whether you should complete any forms beforehand. Bring your worksheet in the format that works for you: printed, saved on your phone or copied into a notebook.

Under-eye consultation questions, answered

What should I bring to an under-eye consultation?

Bring your written concerns, questions, relevant treatment records and a list of medical information to discuss privately with the clinician. Ask the practice about its specific intake requirements when booking. If a product name or earlier treatment date is unknown, say so instead of guessing.

Do I need to choose a treatment before the appointment?

No treatment choice is required to use this worksheet. Its purpose is to help you describe your concern and record the assessment and options discussed. Ask the practice what type of appointment fits that purpose when you book.

Can I use this worksheet if I have had previous under-eye treatment?

Yes. Use the history section to organize the treatments, dates and records you know about. Tell the clinician what remains uncertain and ask what additional information they need before discussing a recommendation.

How should I compare two consultation recommendations?

Compare the concern each clinician assessed, the proposed option, its intended purpose, limitations, risks, alternatives and follow-up arrangements. Ask about differences you do not understand. An identical treatment name does not explain why either clinician recommended it for you.

Does this worksheet replace medical advice?

No. It helps organize a conversation and record the information you receive. It does not diagnose an under-eye concern, select a treatment, provide aftercare instructions or predict results. Your treating clinician must address your individual assessment and care questions.
